The eerie world of Pathologic returns with Pathologic 3: Quarantine, a free prologue to the upcoming Pathologic 3. Publisher HypeTrain Digital and developer Ice-Pick Lodge have announced that the game will launch on March 17 for PC via Steam.

A scientist, a plague, and a town full of secrets

Set before the events of Pathologic 3, Quarantine follows Daniil Dankovsky, a renowned doctor from the Capital, in his search for immortality. His work is frowned upon by those in power, but a rumour about an immortal man in a remote steppe town could be the breakthrough he needs—if he can reach him.

As he arrives, the town is gripped by a deadly plague. Dankovsky must examine patients, uncover hidden truths, and issue decrees based on his findings. His perspective shapes the story, but his persecutors see things differently. Can he change his own fate by rewriting his past?

Pathologic 3: Quarantine key features

  • A gripping narrative – Step into the mind of Daniil Dankovsky as he navigates a town teetering on the edge of catastrophe.
  • Unreliable narration – Experience events through his perspective and the accusations of others. What really happened?
  • Investigative medicine – Diagnose patients, explore their lives, and make tough choices to combat the Sand Plague.
  • Mind over matter – Everything you see tells a story. Connect the dots, analyse your surroundings, and uncover the truth.

Dark themes and mature content

Pathologic 3: Quarantine doesn’t shy away from unsettling themes. As a doctor, players will examine patients, with some nudity shown through the interface, and encounter blood, injuries, and disturbing imagery. Strong language and substance use may also appear depending on the narrative.
